South Riverdale Community Health Centre is a non-profit, multi-service organization that provides primary healthcare, social and community outreach services with an emphasis on health promotion and disease prevention primarily to people of East Toronto. The Community Health Worker – Mobile Outreach will operate mobile outreach services within the defined catchment area of the COUNTERfit Harm Reduction Program in accordance with established protocols and guidelines. Work will be carried out on evenings and weekends. This position is responsible to deliver supplies to the drug using community in East Toronto.
This position is expected to work onsite.
Responsibilities:
• Deliver harm reduction supplies to various locations of service users using a harm reduction framework and a client-centred approach in East Toronto between the hours of 6 pm and 12 midnight Monday through Sunday
• Identify and liaise with services in east Toronto to provide coordinated and effective means of connecting with clients wherever they may be, and delivering service (including evenings and evenings)
• Keep a log of the time calls are received, the time in which they leave to fulfill requests, and the time they arrive to the specified location
• Call the Mobile Phone Backup Worker to advise when leaving for a delivery and when delivery is complete
• Return used needles to South Riverdale Community Health Centre utilizing universal precautionary measures and enter the returns into the Neo database
• Follow organizational policies and safety protocols, including needle distribution and collection
• Provide harm reduction education including HIV & Hep C-specific prevention materials to people who use drugs to support effective referrals
• Provide referrals and support to people who use drugs as necessary including referrals to medical & legal services, treatment programs, income assistance, mental health services and other community resources
• Maintain records, reports and other statistical information and produce reports as required
• Work collaboratively with colleagues from all sectors of health and social services to increase knowledge exchange and improve referral pathways
• Participate in program evaluation and client needs assessment, in conjunction with the Program Manager, Harm Reduction and Hepatitis C
• Inform the Program Manager, Harm Reduction and Hepatitis C of any incidents that have occurred during work hours using the incident reporting procedure
• Conduct mandatory vehicle circle checks prior to deliveries, noting minor and major vehicle defects, topping up vehicle fluids (e.g. gas and windshield wiper fluid) in a timely manner
• Report to Manager any concerns regarding the condition of the vehicle which may make the vehicle unsafe to drive
• Ensure the vehicle is clean and presentable
• Complete all documentation and reports as required by legislation or organizational policies and practices in a timely and efficient manner
• Provide the Program Manager, Harm Reduction and Hepatitis C with time sheets monthly
• Participate in regular supervision with Program Manager
• All other duties as required
Qualifications:
• Post-secondary education from a recognized University or College in a related field i.e. counselling, addictions studies, adult education, or equivalent work experience
• Experience in social services or related field preferred, particularly in areas of crisis de-escalation and conflict resolution
• Requires a Driver’s Abstract for a minimum of 3 years, Driver’s License Check, valid driver’s license, personal insurance (if have personal auto insurance)
• Requires Consumer AutoPlus, or, a letter of experience from insurance company and/or from a previous employer’s insurance company confirming at least three years driving a car/van with a G driver’s license
• 3-5 years’ experience working with people who use drugs and/or working with people with blood borne diseases
• First Aid & CPR (BLS) training
• Overdose response training
• Experience in training and delivering safer sex and safe drug use information
• Must be experienced in dealing with people who use drugs
• Must have demonstrated experience and understanding of harm reduction practices
• Working knowledge of HIV/AIDS from a harm reduction perspective
• Safe and cautious driver
• Must comply with and complete all necessary drivers training as per organizational policy
• Proficiency in the use of computer technology and various software applications
• Comfortable using technology such as a smartphone and GPS
• Ability to function as a member of a multi-disciplinary team
• Excellent interpersonal and organizational skills
• Demonstrated reliability and dependability
• Strong oral and written communication skills
• Strong problem-solving skills with the ability to exercise excellent judgement
• Demonstrated respect for diversity, equity, anti-racism and accessibility with the ability to work collaboratively and positively with others
• Ability to work effectively with people who use drugs and knowledge of relevant social, health and treatment services available for People who Use Drugs
• Must have knowledge and be comfortable working with, people who use illegal drugs
• Must be sex and drug positive
• Demonstrates compassion and respect for people who use drugs
• Track record of reliability and dependability
• Resident of East Toronto (knowledge of geography streets, etc.) an asset
• Fluency in a second language is an asset
• Police clearance documentation will be required for employment
• Proof of triple COVID-19 vaccination status will be required for employment
