This program matches individuals with trained mentors to provide one on one support based on their lived experience with Parkinson’s, either as a person living with Parkinson’s disease, or as a care partner. This connection provides a safe place to brainstorm, share ideas and connect with someone who has similar experiences. You MUST have lived experience as a person living with Parkinson’s Disease or as a Care partner.

The Intake Coordination team is responsible for gathering all of the necessary information needed to ensure that our moves are as safe and smooth as possible. This involves answering emails and phone calls, and collecting information using provided templates and forms. The Intake Coordinator reports to the Intake Supervisor(s).
