Community Health Worker – Mobile Outreach

The Community Health Worker – Mobile Outreach will operate mobile outreach services within the defined catchment area of the COUNTERfit Harm Reduction Program in accordance with established protocols and guidelines. Work will be carried out on evenings and weekends. This position is responsible to deliver supplies to the drug using community in East Toronto.

Community Resource Worker

The Community Resource Worker works with BQWCHC clients and community members to facilitate access and coordination of the various agencies/providers/services that are involved in the care of clients/community members with unmet health and social needs. The CRW will provide support in the navigation of the health and social services system and development of sustainable strategies for the mitigation of barriers to access to more optimal health and wellbeing in alignment with client goals.
