Nurse Practitioner, Primary Care

Centertown CHC has a Casual opportunity for Nurse Practitioner in our Primary Care Program. Join a dynamic group of health care professionals providing a full spectrum of primary health care in a community health centre located in downtown Ottawa. Our interdisciplinary team includes other nurse practitioners, family physicians, registered nurses, medical receptionists diabetes educators, social workers, community support workers, and other health care professionals.

Registered Nurse – Primary Care

The primary role of the nurse is to preserve and promote the health of clients of the community served by CCHC. Using the skills and knowledge of the nursing practice and following the standards and guidelines of the profession, the nurse provides care to individuals, families and groups within CCHC and in the community. Nursing care is provided within the context of a inter-disciplinary primary care team and recognizes that the social determinants of health affect health throughout life.

Bilingual, Nurse – Early Years and Primary Care Program

The Early Years (EY) / Primary Care Nurse works as part of an interdisciplinary team within the Community Health Promotion and Early Years and Primary Care programs. The EY nurse is responsible to assist in the development, promotion, service delivery and evaluation of programs related to families, newborns, well babies and well children.L’infirmière ou l’infirmier, programme de la petite enfance et des soins de santé primaires, fait partie d’une équipe interdisciplinaire au sein des CCHC.
