Like the unique mosaic at our front door, Sherbourne was shaped by our communities.  As with our programs and services, when the pieces are put together, the different colours and textures form something inviting, vibrant and relevant to the people we serve.  We work closely with our partners and community members. Our primary care and chronic disease management services, health promotion and education, outreach and social supports, and mental health services work together to provide transformative care and support.

While Sherbourne is open to everyone in our neighbourhood, we are particularly committed to helping people who may have been under-served by conventional health care.

Case Worker

Sherbourne Health and The Neighbourhood Organization are the co-lead organizations in the Health Access Executive table with 15 other health and social service providers. The Health Access Expansion Project will focus on expanding the existing Health Access St. James Town model to surrounding neighbourhoods.

Case Manager (Youth Resource Worker)

The Case Manager (Youth Resource Worker) engages in a collaborative process of assessment, planning, facilitation, care coordination, evaluation, and advocacy to meet the comprehensive health needs of Black 2SLGBTQ youth and young adults 14-29.

Registered Dietitian – Chronic Disease Prevention & Management for 2SLGBTQ Communities

We are presently looking for a Full-Time Registered Dietitian to join our team. Reporting to the Senior Program Manager, Community Programs and Capacity Building Initiatives, the Registered Dietitian will work on therapeutic and preventative nutritional assessment, counseling, and for education to individual service users, groups, and communities, and develop and implement health promotion activities that align with Sherbourne Health’s Health Promotion Framework and food security strategy.

Registered Nurse, Acute Respite Care

As a Registered Nurse, you are a team player who is an integral member of an interdisciplinary team to support the improvement of the health and well-being of clients, by understanding the impact of the broad determinants of health on people’s ability to attend to their health care needs. You will provide care within a health promotion framework and will have familiarity with the health issues that are experienced by individuals who are homeless and vulnerable.

Registered Practical Nurse, Acute Respite Care

The Registered Practical Nurse is a team player and an integral member of an interdisciplinary team to support the improvement of the health and well-being of clients, by understanding the impact of the broad determinants of health on people’s ability to attend to their health care needs.
